Pediatric Training Verification

Please submit verification requests for Residency and Fellowship completed in the UCSF Department of Pediatrics to the UCSF Pediatric Training Program via the UCSF Pediatric Verification Form.

Due to the high volume of requests for verifications that we receive, there is an administrative processing fee for each request. Please see below our verification service fee breakdown. Note that processing takes anywhere from 5 to 14 business days.

Payment Information

For payment, we only accept checks payable to: UC Regents and mailed to 550 16^th Street, Box 0110, Department of Pediatrics, Medical Education, San Francisco, CA 94158. Verification services are rendered only after we receive a payment confirmation via email. If you require an invoice, please indicate so in your request. WE DO NOT ACCEPT CREDIT CARDS.
